To all of my Friends and Patients:
For forty years, I have been honored to care for my patients, whom I consider my extended family. However, it is now time for me to pass on that responsibility to the next generation.
Therefore, I will not be seeing patients after June 30, 2022.
My partners and trusted colleagues for many years, Drs. Wade Dickinson and Camila Marquez, together with their excellent nurse practitioners, will continue to provide you with the same level of excellent care I hope I have provided. Patients who continue care with Drs. Dickinson and Marquez will not need to transfer medical records, as they use the same medical record system.
I will continue to answer telephone calls until July 15, 2022, after which all calls should be directed to the offices of Drs. Dickinson and Marquez.
During this transition, be assured your health care will not be compromised, and we will strive to maintain your confidence and trust, giving you the information needed to make the best decisions for your health.

Steps to Obtain Your Medical Records
If you need to obtain your medical records to transfer to another provider, please follow the steps below. Our aim is to make this transition as smooth as possible, and ensure the highest standards of care.
REMEMBER: If you have been my patient and you plan to continue your care with my colleagues, Drs. Dickinson and Marquez, you do NOT need to complete these steps. Your records will simply be maintained in the same system, overseen by Drs. Dickinson and Marquez.
Step 1: Download and Complete Form
Please click the button to download a blank Authorization Form. This form authorizes our office to release your medical records to you or to another provider. Fill out the form as directed.
Step 2: Submit Form
After you have completed the Authorization Form, please submit it to us. You are welcome to bring it into the office until July 15th. After July 15th, you may email it to us. Our email address is: richardcshebelutmd@gmail.com
Step 3: Pick Up Records
Please note: if you directed us to send your medical records to another provider, you don't need to complete this step. We will send your records to the specified provider. If you want your records, we will prepare them within 7-10 days. When they are ready, we will mail them to you at the address that you have provided. After September 30, please follow the directions below.
How to Obtain Your Records after September 30
Until September 30, you can follow the 3 steps above to obtain your records.
However, to obtain your records AFTER SEPTEMBER 30, you will need to contact Drs. Dickinson or Marquez to arrange alternative options. Please contact them here:

Again, I am beyond grateful to have been a part of your lives for over forty years. Thank you for allowing me to serve you.
As I close this chapter of my life, I am heartened by the experiences, lessons, and people who fill my life’s story. And, as I open a blank page to start a new chapter, I am optimistic for the story yet to be told.
